Navigation may take the form of:
- Accompanying clients to appointments, hospital stays, and at-home service calls. Providing point-of-care guidance.
- Collaborating with the care team, including other case workers, family members and health proxies.
- Acting as a liaison between healthcare providers to ensure medical chart information is accurate.
- Helping to identify service providers and extracting price estimations when possible.
